Existing Client RX Food and Medication Refills Request Form
Click the “RX Refill” request button below to submit a refill request for prescription food or medication.
NOTE: You will be able to schedule a “pick-up” appointment day and time. Seven days after refill request, if not picked up, the items will be returned to stock.
Existing Client Appointment Booking Form
To book a medical appointment for an exam with a Doctor or a vaccinations/procedure with Nurses, please click the “Book Appointment” button below. This includes follow-up Medical Progress Exams with the Doctor, as well as Nurse services, such as an Anal Gland expression.
NOTE: For the Office visit field on the form itself, please select Nurse or Doctor from the drop down menu.
